Certified Case Manager (RN, CCM)

Job Summary:
  • Establish and maintain positive relationships with patients, visitors, and other employees.

  • Contributes to and participates in the Performance/Quality Improvement activities of the assigned department.
  • Consistent adherence to the specific rules and regulations of the Bronx-Lebanon Hospital Center (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.
  • Utilizes Care Management to conduct assessments for all admissions to the assigned unit, to determine Care Transition and Discharge Planning needs. Including but not limited to:
  • Poly-pharmacy
  • Medication compliance
  • Controlled substance use
  • Adherence to HIV medications
  • Readmissions
  • E.R. use
  • Functional status
  • ADL’s
  • Provides supervision to the Social Work Assistant with regard to referrals for Home Care, DME, SNF/Rehab and post-hospitalization outreach.
  • Makes appropriate referrals for Skilled Nursing Facilities, Home Care, Durable Medical Equipment and other required apparatus as needed to safely transition out of the hospital setting.

Required Experience:
  • 3+ years of clinical experience in nursing; preferably in hospital case management
  • Experience with InterQual and Milliman Care Guidelines
  • Knowledge of DRGs and GMLOS, Managed Care and Regulatory Requirements preferred

Education / Certification / License Requirements:
  • Master's Degree preferred
  • RN License
  • Successful completion of Commission for Case Management Certification (CCMC) within 3 years of hire if not currently certified.
