Position Purpose:
Oversee intensive case management and therapy services to ensure the quality of services. Serves as the leader to the treatment team and as a representative on behalf of the member and LifeShare.
Job Description:
  • Review and approve bio-psychosocial assessments on behalf of clinical team; review and approve initial and ongoing treatment plans for assigned members.
  • Actively lead and provide clinical supervision to the treatment team, actively manage team case status and progress.
  • Review and approve clinical team’s completed intakes and discharge summaries for all cases assigned.
  • Supervise quarterly treatment plan review process and approve plans as the prescribing practitioner.
  • Act as resource to clinical team and point of escalation with regard to sessions, in-home visits, referrals for youth requiring additional or alternative services.
  • In the absence of an available clinical team member, attend treatment plan review meetings, including school meetings, court hearings, and scheduled therapy sessions as a representative of LifeShare and the families supported by LifeShare.
  • Conduct weekly rounds with clinical team in order to supervise coordination of services for each individual on caseload.
  • Develop and maintain relationships with state and local stakeholders ensuring effective coordination of care.
  • Provide ongoing training and guidance to clinical staff in methodologies and best practices.
  • Provide consultation and guidance to clinical team when crisis situation occurs, ensuring timely response.
  • Master’s degree in social work, counseling, psychology, or a related field.
  • 2+ years of direct work experience with children in the foster care and/or juvenile justice system and their families.
  • Must have one of the following: LICSW, LCMHC, LMFT, LPC, or similar therapeutic license granted by state.
  • Valid driver’s license
